According to the Youth Employment Protection Act, minors are required to undergo certain medical examinations free of charge when entering the workforce. A so-called examination authorization certificate (UBS) is required for this purpose

Apply online:
Go to the online application at www.untersuchungsberechtigungsschein.de, log in with your ID document with activated online ID function and enter the other data.
After submitting the form, you will receive the examination authorization form (UBS) with the so-called UBS ID and the fillable questionnaire to prepare for your doctor's appointment within a few seconds. Both documents can be saved and printed out.
Application on site:
You present yourself at the Citizens' Office for the issue of the examination authorization certificate. You must bring an identification document (identity card, child ID card or passport). You will be issued with the examination authorization certificate with UBS ID and the fillable questionnaire to prepare for your appointment. You must bring these documents with you to the examination.
You will receive a certificate after the examination. You should present this to your employer before starting work.
Deadline
The examination must be carried out before you start work. You must start work no later than 14 months after the examination, otherwise you must repeat the examination. The first follow-up examination must take place between the tenth and twelfth month after starting work if you are still under the age of 18 by then. You must submit the certificate to your employer no later than 14 months after taking up employment.
